Summary of Benefits


As an employee of Sky Angel, you will find great advantages in your benefits package. The company works hard to provide competitive, value-based benefits. Although benefits are not generally considered part of your income, they are worth a lot, and can add considerable total value to your overall compensation package. Our benefits program is designed to deliver protection, security and opportunity.

These benefits will be available to all full-time employees upon completion of ninety days of employment. Sky Angel continually reviews the effectiveness of our benefits package in meeting our business and employee needs. The following is a brief overview of the benefits available to you and your family, more specific information and detailed plan booklets are available through Human Resources.

1. Group Health Insurance
  1. United Health Care is our medical provider for all eligible employees.  Our plan offers 80% co-insurance if the provider is in-network (belongs to the approved PPO organization) and 60% if out-of-network.

  2. Prescription drugs, Mental Health, Select Vision coverage and Wellness Care are some of the included coverages.

  3. Eligibility: Full-time status; 1st of month following ninety days (Ex: Hire date 8/17/09 - benefits begin 12/01/09)

  4. Sky Angel pays 2/3 and employee pays 1/3 for full-time employees and their families.
2. Flexible Spending Accounts
  1. Dependent Care Account – You can elect to have up to $5,000 of pretax money deducted from your paycheck to pay for child/elder care expenses lowering your taxable income and increasing your spendable income.

  2. Health Care Account – As with the Dependent Care account, you can use up to $3,000 in pretax deductions to pay health care expenses not covered under any other plan also lowering your taxable income.
3. Dental Insurance
  1. Our two Guardian Dental PP0 plans ($1,500 or $1,000 annual coverage) allow employees to choose any   dentist in or out of network.

  2. Sky Angel pays for one third of the cost for employees and their families.
4. Vision Plan

Our Guardian Vision Plan provides high quality vision care coverage with significant savings. The Vision exam copay is $10.00 annually, and Material copay is $25.00.  Lenses are no cost and frames are also no cost up to an amount of $120. Medically necessary contact lenses are no cost and elective or replacement contact lenses are no cost up to $120.00.  Premium paid by employee.

5. Company Paid Group Life Insurance
  1. Benefit equal to two times annual compensation

  2. Premium paid by Sky Angel (no cost to employee)

  3. Includes Accidental Death & Dismemberment and Long-term Disability benefit (available 90 days after qualifying event)
6. Voluntary Group Life Insurance
  1. Premium paid by employee

  2. Supplement to Group Life Insurance
7. 401(k) Retirement Plan
  1. Administered by Principal Financial Group

  2. Sky Angel will match 100% of employee contribution up to 4% of the employee contribution based on annual compensation

  3. Eligibility: Must be at least 21 years of age and employed for six months prior to scheduled semi-annual entry dates
8. Educational Assistance Program
  1. Sky Angel will reimburse full time employees up to $2,500 each calendar year to pursue their college education and job related professional development.

  2. Employees will become eligible for this benefit upon satisfactory completion of one year of full time employment with Sky Angel.

  3. Must be accredited degree program or job related courses.
9. Paid Time Off
  1. Paid Time Off (PTO) is earned based on hourly/salary classification; available on accrual basis.

  2. 3 Bereavement days are available for immediate family members.

  6. Premiums paid by the employee through payroll deduction
11. Sky Angel Program Subscriptions

Sky Angel will loan eligible associates a set-top box to use at home and will provide discounted subscription costs.

Employees may choose to subscribe to the Sky Angel Faith Package or the Family Value Pak.  The Faith Package subscription is free of charge and the Family Values Pak is available at the discounted rate of $7.50 per month.

12. Employee Assistance Program (EAP)

Employees and their families have access to seminars and counselors for assistance with issues such as financial issues, stress, depression, alcohol or drug abuse, family relationships, elder care, etc.
